Print your own free EMT Alert

In a home emergency, EMT Alert will speak for you and your family members in your absence or if you are unable to speak for yourself. The EMT Alert is to be printed on both sides of an ordinary sheet of white 8½" x 11" paper. You can choose to print a version with a living will or without one. Click on the links below for the version you wish to print. The EMT Alert requires Adobe Reader to view and print.

1. Print page #1, with or without a living will, then print page #2 on the back of page #1.
2. Trim off 3/4" of scrap paper along the one side of page #1 as shown.
3. Fill in the necessary information with pencil so it can be easily updated later.
4. Fold the sheet on the dotted lines leaving the red and black title page on the outside.
5. Slide the completed EMT Alert into any of our 4" x 6" pocket style magnetic frames.
6. Display your EMT Alert in plain sight magnetically, on your kitchen fridge door.

Your EMT Alert will fit into any of our 4" x 6" pocket style, magnetic frames, although they slide most easily in and out of our 4" x 6"Hard Pocket style holders.
